Default Rev limiter or transmission

Hey guys, I have an 04 Maxima SL with the 5 speed auto and I need advice fixing it. It was a fantastic car when I got it at 100K miles. Unfortunately, it became my wife's car and has been a pest for the last five years. I've kept it going and have done many repairs on it under the gun without a spare car. Now, she drives something else and I have the opportunity to make it nice again.
The problem I have with it seems to be pretty bad. It is extremely sluggish, slow to into the next gear and pretty much has no useful power over 2k rpms. If you floor it in any gear and at any speed the rpms will go to 4k and behave like its hitting the rev limiter! It has had codes 1283 and p1800 for quite awhile but no other codes. I'm at a loss on how to fix this and don't want to waste and money throwing parts at it. Has anyone had this problem, what was the solution? Thanks
